Life as PRP Part 1 (Inpatient)

4 November 2013 was my Hari Peristiwa, when i was officially registered with Government as a Public Server. My parents and I went to JKN Kedah at Alor Setar to register. There I was asked to give the original offer letter from KKM to the one of the staff. He then put a stamp on my letter which said "Lapor diri di PKD Kubang Pasu & menjalani penempatan siswazah di HSAH". I was a bit confuse earlier so do the staff at PKD KuBang Pasu. Rupanya I was given the post of Gred U41 at PKD Kubang Pasu since the post of U41 in hospital was occupied due to newly finished PRPs (whom still floating ATM). Hence, I have 2 employer now.

So things in PKD just settled on the evening so I was unable to rush to HSAH for lapor diri on the same day so I went HSAH on 6th. There, i met up with other PRPs, 3 from UKM, 1 from CUCMS, 1 from UITM and me from IIUM. On 6th & 7th we just having briefing from respective departments such as satellite, outpatient, manufacturing, ward etc.

10th November, we pick a random number as been asked by our preceptor. From the number, we knew the flow of departments for our one year PRP.  I chose No. 1, so I was assigned to start with Inpatient department, followed by TPN, and CDR. Meanwhile the other PRP chose No.2, so she have to start in Outpatient department, followed with IP and the list goes on.

1st day in IP, im doing nothing much. The Pegawais explained the jobscopes of PRP, the location of drugs etc. Since in IP, we have to verify new prescriptions for polypharmacy, wrong dose, wrong drug, wrong frequency, then check on regular prescriptions list. For inpatient department, we just have to supply the medicines to the patients in wards. The term "Regular" is for the few days admitted patient, and "New Prescription" normally use for new admission patient, changing drugs, and reorder drugs.  If there is anything that seems wrong, we must do intervention by calling the doctor. We only allowed to call the doctors after 10 am after they finish ward round. There is some wards incharge by pharmacist, so, for that, its better to consult with pharmacist in charge for that specific ward rather than calling the doctor first.

Besides, we also do the ward trolley (if PPF was not there), check the trolley for polypharmacy, wrong medications etc. The staff nurse will send the empty trolley (sometimes with returned medicines) to IP on early morning and about 12 pm, they will come down and pick up the fulfilled ward trolley.

Aside from that, the PRPs have to do counselling and its better if you validate first. Earlier I was following the senior PRP to perform the counselling and they watch me do it. And now I able to do perform by myself. Even so I still have to validate my counselling with the Pegawai that I was assigned to. The counselling in IP covers MDI/Accuhaler/Easyhaler/Handyhaler, Insulin pen, CRP (Cardiac Rehabilitation Programme), and Warfarin (which I haven't encounter yet).

Other than that, the PRPs also have to do the extemporaneous preparation and DD supply. For DD, I never have the chance to do it betul2 yet, but able to try once under the guidance of my PRP's friend.

Overall, there goes my almost 3 weeks in IP. It sounds easy right? Actually, it was not. ^_^
The situation may be varied according to you, the people you're working with, and the place itself. But I really enjoyed my days in IP, great Pegawais, great PRPs seniors, great PRPs friends, and great staffs.

This coming December wont come easy, my next station is to TPN (still near IP), and will be starting the oncall, dispensing slot, and flexi. Just wish me luck okay =D

Prosedur Lepas Tamat Belajar Pharmacist Gred U41


Notes : I started to write this post since 29th August 2013, but I haven't yet to receive the full documents regarding the procedures, so I hold this post for a while before I publish it until the 1st day of the briefing or whatsoever thus I can tell the exact procedure to you. It means the complete procedures take about 3 months since the released of our results on 19th June 2013.

I've always wanted to post these things so that I won't forget. So here the thing,

This entry is about the steps to be taken after completed studies, for SPA applicants. Perhaps the procedure is about to be the same with other local universities and the overseas students as well.

1) LAPOR DIRI TAMAT BELAJAR + CLEARANCE
1st: Awak akan dapat surat JPA yang memerlukan awak untuk LAPOR DIRI TAMAT BELAJAR. Boleh email/ post/ give directly to the office/ online form dari website JPA; salinan semua keputusan peperiksaan semester  ( since you not yet to receive the full transcript) + a complete filled in LAPOR DIRI form + Surat Permohonan Perlantikan Pekerjaan SPA ,

2nd: you should do University Clearance to make sure you left nothing unsettled with the university such as fines, unreturned keys, books, etc. For IIUM, we have to do clearance and settle with 3 departments, Finance department for the studies fees + fines (if any), Library for any unturned books + fines, Mahallah (hostel). You can just call each department or even drop by to each departments itself in Kuantan.

2) FULL TRANSCRIPT
Complete Clearance, then we post our student's card to be punched (making it invalid) + graduation fees (for those yang still tak settle the fees - RM 130 for IIUM) to the One Stop Student Centre. Then we will receive our Full Transcript.

Punched matric card + Transcript + Letter of completed studies endorsed by Senate

3) PHARMACY BOARD
Print out Borang 1 (Application for PRP) and complete the document with the requirements below:
a) 1 set Borang 1
b) 1 salinan Ijazah/ Transcript
c) 1 salinan IC
d) Sampul surat beralamat sendiri + Setem RM1 + Pos Berdaftar RM 2.20
Salinan (b) dan (c) disahkan pegawai kerajaan Gred A. Post the documents to Pharmacy Board, according to the address written at the back of the form.

Permohonan Pendaftaran Bersyarat

After few weeks , you'll received this:
Sijl Pendaftaran Bersyarat, Checklist, and Surat Penerimaan.
In the same time you'll also received Surat Pelantikan Pekerjaan from SPA in which you need to do medical checkup, akuan sumpah etc :
Surat Terima Pelantikan + Surat Akuan Sumpah + Borang medical check-up + Surat Aku Janji
Surat Terima Pelantikan diterima dalam 3 salinan. Jadi, 2 keping perlu di bawa semasa lapor diri dan 1 keping lagi WAJIB di pos kepada SPA untuk salinan mereka.

So in total, you'll received 3 letters:
a) Transcript from IIUM
b) Sijil PRP from Pharmacy board
c) Pelantikan Pekerjaan from SPA


Now, just waiting patiently and anxiously for Placement Letter from KKM. Dalam masa yg sama, saya cuba buka akaun KWSP, tak perlu nombor slip gaji lagi, just bagi IC untuk di scan. Kalau yang IC xleh detect tu dia nak salinan IC lah. FOC. Tak perlu bayar apa2. Then tangkap gambar passport around 20 keping as preparation ^o^

Dalam masa yang sama UIA kena prepare untuk convocation pulak bulan November ni. Graduates kena complete tracer dan bawak salinan tu masa nak ambil robe. Untuk mendapatkan multimedia card tu, memang kena submitlah semua ni. Boleh mintak wakil ambilkan kalau tak free. Cuma kena isi satu borang extra.

Ok berikutnya, lebih kurang selepas 5-6 minggu dari tarikh surat terakhir sampai, surat KKM pulak menjelma. Melalui pos laju. So get prepared pakai tudung semua tunggu depan pintu pada hari tersebut. So dah dapat surat KKM.

 


Dan ni senarai dokumen yang perlu dibuat, maka, selamat mendaftar ! 


Ambik, betapa lamanya proses nak start kerja hah, dari bulan June, habis final hingga ~~~~ October, dan lapor diri awal November =) So, cari lah kerja part time ye ! Tak pon, plan lah cuti tu betol2 kalau tak rugi rasa. hehe =p

Summary As a Student


Hai ! Kak Dela? *"Apa Khabar?" in Russian. That greeting actually stuck in my head cause it reminds me on how my cousins entitled me, hehe. Its been a while since I've updated my virtual diary here. Well, I'm currently on holiday now. It seems my initial tertiary education in IIUM Kuantan had ended. And it is still undecided whether should or shouldn't i further study. My last paper as undergrads was on 6th June 2013.

I stayed in Kuantan until 11th of June cause my batch had Annual Grand Dinner in Hyatt Hotel , at Teluk Cempedak on 10th June 2013. It was great, thanks a lot for those whom were working hard to make sure the program went well and it was. Some of the lecturers also came to see us for the last time. Terharu~~ And we, the sister performed 3 songs specially dedicated for friends and lecturers.

So now i'm jobless, and enjoy the last Ramadhan as student before the hectic work life begins.
Overall, throughout those 6 years;  4 years together with friends in Kuantan, and 2 years in Petaling Jaya (CFS), where I thought it was a very long time to finish study BUT a really short time to value each other, I truly enjoyed those moment and thanks a lot for the happy, sour, sad, bitter, cheer, joy time we spend together, all those skema acts, nakal2 acts, transform jubah ke jeans when outing, sama2 marah bila kena paksa pakai matrik kad when outing, celebrate birthdays together, gossiping, karaokae-ing until the voice out, laugh til we cant breath, heart to heart talk, fight, late night lepak on beach , zumba-ing, it was all too precious.

Sorry for everything and I do really pray that each of us will success in our future undertaking. Since our batch claimed to be as "don't be like batch RX8" , I hope we can see something great within us to be polished, and to prove that they are mistaken. Best of luck in your life ! #RX8

SPA interview for Pharmacy students




Salam, this is the post of my experience on being interviewed by SPA recently. I bet most of JPA holders will sooner or later be interviewed by SPA. The interview session for 4th year Pharmacy students (RX8) IIUM was conducted on 17-19 April 2013 located in our beloved building, Kuliyyah of Pharmacy. According to the name list, we were divided into small group with respective interviewer. My name was seen at number 7 out of 10 for the last day of the session which was on Friday. We were arranged according to birthdate. 

For my group, we got Mr. D (SPA Rep) and Mrs. F (Pharmacist) as our interviewer. They are quite relax actually but we aren't. I entered with 2 friends. As we step in, we gave our folder of certs plus resume, and our request placement form. As per usual, we have to introduce ourselves; name, hometown, family background, and what you want to say bout yourself( involvement in coco, or quality in yourself for example). As for me, i summarize my involvement in coco such as my participation in charity under supportive unit in UIA. Then Mr. D asked bout my educational visit to Australia. This part, i just tell things that came in mind at that time. Sincerely i dont really remember things we do in Aussie.

The interview started with Mr.D. He asked us to tell what do we understand bout TCM (traditional complementary meds). We give acupunture as example, then he asked again, how do we think the acupunture can improve health ( not sure exact question but approximately like this).He asked for other answers, we said bekam and so on. But we weren't able to mention gingko biloba, or any other herbs at that time despite he was asking bout medicines in general. Nervous i guess.

I dont really remember the sequence of the questions, but from those two, we were asked bout Blue book, What is Blue Book? What is the contain of the book? The pharmacist asked us a real-situation case' question, what if the doctor want to suggest or give the medicines other than written in the book? what should we do? To be exact is the procedures. The answer is to refer to Pengarah Hospital and fill in a specific form to request for approval to dispense that sort of medicines  etc. 

The pharmacist asked bout AMR (antimicrobial resistance) . How can we detect if the antimicrobial is resistance, and the answer is C&S (culture& sensitivity). Well, when you know the reasons behind the infection and able to identify the microorganism presence then you will know which antibacterial to be given. Right? TDM can be true as well. But not to check for microbial resistancy but more to toxicities, or incompliance. It is specifically use for drug monitoring procedure.

Before that, Mr D asked about rawatan Islam since this method is always be an option nowadays. How can we think rawatan Islam such as Al-Quran recitation can help in treating patient? I answered it by giving example from the article that i read in a website regarding how the water molecules can become neatly organized, beautifully after hearing the Quran recitation.Since about 70% of body consist of water hence the organized water molecules inside our body may trigger the freshness inside, and spiritually. Thus, our body tend to feel better. That is one of the beauties of Quran.

The pharmacist asked how to counsel Muslim patients especially in Ramadhan (fasting month) since the patient usually skip fasting just to ensure they took the medicines. Actually for tds dose, patient can take the meds during sahur (sunrise), bukak puasa (sunset) and during moreh(supper). Meanwhile for the usage of drug containing porcein or bovine sources, the pharmacist should find other alternatives and only if the patient is critically ill and need the meds then those meds can be given to the patient( should read other resources for more confirmation). 

Like other common interview, we should know and memorize the name of important person in BPFK Chart. The Ketua Pengarah Kesihatan and who is the Ketua Kanan Farmasi in full name. And, Health Minister too.

That's all i can recalled, but no worries, the interview is more to discussion and they just want to hear your opinions. Hope this sharing may help a bit. Best of luck and wish me luck as well !

New Year Wish List !


SALAM, It's the 2nd month of 2013 today. For almost 4 years I never felt any excitement of New Year because since this past 4 years, we had final examination which started by the end of December and only finished in the middle of January. Oh hey, Happy New Year ! Like every other years, everyone will normally do a wish-list of the year or azam tahun baru what we called in layman language. So what's yours? Well for me, I really-really want to finish up this Degree (B.Pharm) without any extended semester. Since this is the 1st lecture week of our final-sem-gonna-be, I felt this sem is gonna be tough. Total up, I'm going to take 15 credits for this semester. Secondly, after finish up studies, I wish to get the job as soon as possible. I don't even mind where will I begin the PRPs (housemanship) as long as I've given the placement. But my parents do really hope that I could work in Sg. Petani. I'm OK with that too. Indeed beforehand, we will be having some tough moment in this semester which includes industrial VIVA and SPA interview.  Thirdly, I want to get married or at least engaged to errr don't know who yet. Fourthly, I want to travel in Malaysia with my siblings ! We've dream of it for a very very long time ago. Earlier, we've planned to Japan but looking at our current economical status now, I guess holiday in Malaysia itself won't do much harm. Whatever wish-list I had, I just wanna be graduated first ! May Allah ease the way. Please please please pray for us all.
